The amount of perks available in each Territory is governed roughly by how close it is to the center. Ramafa in this case is right on the outside and the perk it has grants x5 random Medical items at level 1 per day, this has now leveled up to level 3, doubling the reward twice to x20 per day. Whereas Senda was awarding x10 Medical items per day at level 1, so is now awarding 20 per day at level 2.
