Being attacked even when online is one of the reasons why we got millions of accounts that they are not playing any more attacks are accepted during wars so you better add enlist mode on cartels then it will players options to join cartel 

 That wars or the one for revives or not to join any cartel in that way no one can be annoyed in the game but in this way we get bored and endup not playing

 Imagine logging in and you in hospital you use meds and get attacked again while online until you in medical cooldown max then you stay in hospital and cant do anything how are you going to feel
